Why Do Immigration Medical Exam (IME) Fees Depend on Age?
You may be wondering: “Why does the price of the immigration medical exam go up as you get older?” If you’re booking your appointment for yourself or your family, you’ve probably noticed that the fees aren’t the same for everyone. Here’s everything you need to know about how age affects the cost and why it does.
The cost of your exam is based on the number and type of tests required by Immigration, Refugees and Citizenship Canada (IRCC). The older you are, the more comprehensive the exam becomes. That means more tests, more time, and a higher cost. The explanations for our fees are broken down by age group below. Please note all fees stated below are weekday rates and are taken as of July 24, 2025 and are subject to change in the future.
Ages 0–10: The Basics
- Younger children are at a lower risk for certain health conditions, so the exam is simpler and quicker. No extra tests are needed.
- What’s required: Only a physical exam.
- No chest x-ray, no blood test.
- Total cost: $140
Ages 11–14: The Basics + Chest X-ray
- Once children turn 11-years-old, IRCC requires a chest X-ray to screen for tuberculosis and other lung conditions. This adds another cost to the process.
- What’s required: Physical exam + chest x-ray.
- No blood test.
- Total cost: $140 (our clinic fee) + $40 (chest x-ray) = $180
Ages 15–59: The Full Package
- Adults need a more thorough check, including lab work and imaging. This means more time with the doctor, more staff, and more resources are involved in the medical exam.
- What’s required: Physical exam, chest x-ray, and blood tests (for HIV, syphilis, creatinine).
- Total cost: $150 (Physical Exam) + $30 (blood test performed in our clinic) + $40 (chest x-ray) = $220
Ages 60–74: Extra Attention for Seniors
- For clients in this age category, it is more common to develop certain health conditions and to take medications for preventing/managing them. That means the medical exam for this age group takes more time and care. Our doctors spend additional time reviewing any ongoing treatments, chronic conditions, and current medications to make sure your file is complete and accurate for IRCC. This extra attention is reflected in the fee.
- What’s required: Physical exam, chest x-ray, and blood tests (for HIV, syphilis, creatinine). The exam is more detailed in order to address various age-related health risks.
- Total cost: $170 (Physical Exam) + $30 (blood test performed in our clinic) + $40 (chest x-ray) = $240
Ages 75 and Up: The Most Comprehensive Exam
- For seniors, IRCC requires a cognitive assessment in addition to all the standard tests. This takes more time and expertise, so the cost is higher.
- What’s required: Physical exam, chest x-ray, and blood tests (for HIV, syphilis, creatinine), plus a memory (cognitive) test.
- Total cost: $220 (Physical Exam) + $30 (blood test performed in our clinic) + $40 (chest x-ray) = $290

Sometimes, IRCC may request additional tests or specialist reports after your initial exam. This is called a furtherance. If this happens, there will be extra charges for the follow-up visits, any new tests ordered by IRCC, or referrals required to complete your medical file. Furtherance fees typically range from $50 to $100, depending on the additional testing needed.
